Thank you for choosing the MedStar NRH Rehabilitation Network for your outpatient rehabilitation needs. It is our pleasure to serve you. In order to make your first visit go smoothly, please see the information below:


Before your first visit

  • Wear comfortable clothing and plan to arrive 10 minutes before your evaluation appointment

  • Bring your physician's prescription form (and/or Referral Form if required by your insurance)

  • Bring your insurance card and photo identification

  • Download, print and complete the forms below:

Most products provided by these health plans, most commercial carriers and worker’s compensation cases from the surrounding jurisdictions are accepted.  Insurance authorization is conducted prior to scheduling of services.  We welcome you to call one of our outpatient locations for assistance.

*Medicaid: DC Medicaid is only accepted at MedStar NRH Irving Street. When the D.C. Medicaid is managed by a contracted payer it is accepted at all locations. MD Medicaid is only accepted at MedStar NRH Irving Street. When the MD Medicaid is managed by a contracted payer it is accepted at all locations, except when the patient is under 21 and requires therapy. Those cases revert to regular MD Medicaid and may be seen at Irving Street. Please refer outpatient billing questions to toll free 877-558-8549 between the hours of 8:00 a.m. and 5:00 p.m.
